Primary Care for all ages

From annual physicals to ongoing management of chronic conditions like thyroid issues, high blood pressure, and diabetes, we provide comprehensive care for the whole family.

Preventative Care & Lifestyle Coaching

We focus on keeping you healthy, not just treating illness. That means working with you on nutrition, movement, stress, sleep, and other lifestyle factors that impact overall health.

Extended Visits

Appointments are never rushed. We take time to truly listen, understand your concerns, and build a plan that fits your life.

Care Coordination

We help connect the dots between specialists, imaging, and labs. You’ll always have guidance and a point person in your care.

Virtual Care

Reach out by text, phone, or video for refills, quick questions, or follow-up visits. Most issues can be handled without an office trip. But, we will always be happy to see you in office!

Simple in-office Procedures

Services like skin lesion evaluation, stitches, wart treatment, and joint injections are often included at no additional charge or provided at clear, discounted pricing.

*Not included: Emergency care, hospital stays, specialist fees, imaging center costs, most prescription costs, and third-party services. We’ll help you navigate cash-pay or insurance options for these when needed.

  • Children (0–24) … $50 / month

  • Adults (25+) … $80 / month

  • Couples … $150 / month

  • Families (2 adults + up to 4 kids) … $250 / month

 FAQs

  • DPC is a membership model for primary care. Instead of billing insurance for every visit, you pay a flat monthly fee for unlimited access and highly personalized care.

  • No. DPC is not insurance. Keep coverage (or a healthshare) for hospitalizations, surgeries, imaging, and specialist care.

  • We do not bill insurance for our services. You can still use insurance or cash-pay for prescriptions, labs, imaging, specialists, and hospital services. We’ll help you find the most affordable options.

  • Sometimes. HSA/FSA eligibility can vary by plan and current regulations. Check with your HSA/FSA administrator. Beginning in 2026, you will be able to use your HSA for your DPC membership!

  • We offer transparent, cash-pay pricing for common labs and select in-office medications when available. We’ll always tell you the cost up front and help you compare options.

  • We do not bill Medicare. However, we do accept patients 65 years and older.

  • Members get text/phone access and same/next-day visits for urgent needs. For emergencies, call 911 or go to the ER.

  • Yes. We coordinate with your existing specialists and share records as needed.

  • Common in-office procedures related to primary care are typically covered by your membership. If something falls outside of membership, we’ll quote pricing in advance.

  • Membership has a minimum 6-month commitment. After that, cancel anytime with 30 days’ notice.

  • Yes. To preserve access and longer visits, we cap our panel size. If we’re full, you can join the waitlist.

  • We see patients from birth through age 64. Children’s pricing applies when a parent/guardian is also enrolled.

  • Our clinic is in Sloan, Iowa (in the former Nested Boutique space). Parking is easy and we run on time.
